*This is an Office - Based Position*
Overview:
• Reports to Director or Regional Director - Community Health
• Support development of periodic Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP)
• Assist in developing and implementing key initiatives to successfully achieve goals of the CHIP.
• Represent Mercy in local collaborative efforts with community agencies and partners.
• Coordinate local community benefit data collection and reporting.
• Monitor and track data and metrics that demonstrate the impact of community health initiatives.
• Oversee local Community Health Worker program, directly supervising CHWs in ED, clinic, and hospital.
• Manage and/or provide technical assistance of local grants (as needed) for community health services and patient assistance as applicable.
Qualifications:
Experience: 2-3 years' experience in community health or related field.
Education: Bachelor's degree in public or community health, healthcare, health education or related field required.
Certifications: None
Other: Must understand English both written and spoken, must be able to safely and successfully perform job-related functions, with or without reasonable accommodations required by federal, state, or local law, must be able to travel between Mercy communities.
