House Lead for ISL (DMH Residential Services)

• Oversee general household operations• Manage and submit home direct care staff schedules• Manage call outs and reschedule as needed• Monitors DPNS at a minimum of 3 times per week and works with Program Manager to retrain staff as needed.
• Monitor the MAR for errors and work with Community RN and the pharmacy to ensure effective medication management• Ensures the calendar in the MAR software is kept up to date with appointments and events• Ensures appointments are kept.
• Ensures the household Is HCB compliant and Due Process is in the forefront ensuring all rights are made available to consumers.
• Work with Service Coordinator to ensure the Personal Plan outcomes are being met.
• Submit Milage logs, census logs, monthly reports and personal spend ledgers are submitted to office by the 5 of each month.
• Direct plan implementation for individuals as needed• Monitoring implementation outcomes• Oversight/coordination of all the person’s programs and services being received• Submitting EMTs into CIMOR and notifying main office and/or Community RN• Ensuring monthly emergency drills are completed.
• Maintaining a personal property inventory on all consumers under you.
• Sign up and follow Division of Developmental Disabilities Email Blasts • Attend at least 4 provider regional office meetings a year.
• Quality Enhancement Monitoring and Establishes Information Collection Systems.
• Completes Coaching forms and positive interaction reports and submits to the office Job Type: Full-time Pay: $19.
