*A Hospice Aide (HA) is responsible for providing patients with routine daily nursing care in accordance with the patient's assessment and plan of care*, and as may be directed by the department director or supervisor.
Reflects the value that the patient comes first.
*EDUCATION, EXPERIENCE AND/OR TRAINING:*
* Must be a Certified Nursing Assistant in accordance with laws of the state.
* High School diploma or equivalent desired.

*DUTIES:*
* Ensures patient care is provided in accordance with plan of care, speaks with the patient before and while providing care.
Knows and respects the patient's preferences.
* Observes and describes patient status.
Reports pertinent changes to the supervisor and in documentation so the plan of care can be appropriately updated.
* Transfers patient using appropriate safety equipment and procedures.
* Documents care given and patient condition using the appropriate documentation systems.
* Follows established universal precautions, safety and sanitation protocols, safe food handling practices and regulations.
* Upholds patient rights and confidentiality.
Creates and supports a physical environment which is accessible and supports privacy, independence and comfort.
