" Overview: Responsible to oversee various components of Heart Failure (HF) Patients.
This includes but not limited to monitoring and addressing HF patient needs assessment, education assessment, provides coordination of care across the continuum, and incorporates teach-back education technique with patients.
Develops and maintains rapport with those primary and secondary diagnosis of heart failure that are high risk for readmission.
Positively articulates role of HF navigator to all members of the care team to influence organization change.
