*Position Description*
The Nurse Practitioner In-Home Assessment is responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions.
As a Nurse Practitioner In-Home Assessment Per Visit you will:
* Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
* Educate and counsel patient and family based on assessment results, including recommendations
* Performs follow up visits in transitional care
*Qualifications*
* Current RN and NP licensure in the state of practice
* Current CPR certification
* Must be enrolled in both Medicare & Medicaid
Pay: $145.
00 per hour
