As our Quality Care Coordinator - LPN, you'll collaborate closely with our Director of Nursing to deliver top-notch care to our clients.
Your responsibilities will span from providing direct care to overseeing and guiding our dedicated field staff, ensuring that our services meet and exceed regulatory standards.
From conducting assessments and developing personalized care plans to offering counseling and support to clients and their families, you'll be at the heart of our mission to provide exceptional care, every step of the way.
WHAT DO WE NEED FROM YOU?
We're looking for a Quality Care Coordinator - LPN who can work a full-time Monday through Friday schedule, with additional flexibility to meet the needs of our clients, and has the following qualifications:
Current LPN license in the state of Missouri
1+ years of nursing experience
High school diploma or GED
Ability to provide direct client care, supervision, training, and general administration
Ability to adhere to HIPAA and maintain client confidentiality
Ability to read, write, speak, and understand English and communicate effectively
Physical ability to lift up to 50 lbs, stand, walk, squat, bend, kneel, reach, twist, push, and pull (varied assistance is required depending on the mobility of the client)
Willingness to travel up to 40 miles one way
Valid driver's license with an insured and reliable automobile
